Becoming a member of the CMA entitles you to advice on the condition, copies of regular newsletters and the CMA's support network. Please print and return the form below with a cheque made payable to the 'Cardiomyopathy Association'. We will contact you annually to give you an update on our work and achievements in the past year and invite you to make a donation of your choice to support our work. Our online membership scheme uses WorldPay. We accept Visa, Delta, Switch, Mastercard and JCB cards.
