CMA Donations

Your gift makes the difference

You’re the reason we can keep providing our vital services, supporting thousands of people affected by cardiomyopathy.

Thank you for your support!

If you would like to make a regular donation, we suggest doing so by Direct Debit.

At least 1 in 500 people are affected by cardiomyopathy. Every day we provide information and support to people affected by the heart muscle disease cardiomyopathy.  Your support can help us reach more people: helping patients, supporting families, improving care.

The Cardiomyopathy Online Donation scheme uses WorldPay. We accept Visa, Delta, Switch, Mastercard and JCB cards.

This is my own money
This is money I have raised from others for the CMA.

Tick this box and the Government will top up your donations, providing the Cardiomyopathy Association (CMA) with an extra 25p for every £1 you give. And it won't cost you a penny!

Please treat as Gift Aid donations all qualifying gifts of money made today, over the last four years or in the future. I confirm I have paid or will pay an amount of Income Tax and/or Capital Gains Tax for each tax year (6th April to 5th April) that is at least equal to the amount of tax that all the charities or Community Amateur Sports Clubs (CASCs) that I donate to will reclaim on my gifts for that tax year. I understand that other taxes such at VAT and Council Tax do not qualify. I understand the charity will reclaim 25p of tax on every £1 that I have given.

I am not a UK taxpayer

If you pay Income Tax at the higher or additional rate and want to receive the additional tax relief due to you, you must include all your Gift Aid donations on your Self Assessment tax return or ask HM Revenue and Customs to adjust your tax code.

Please notify the charity or CASC if you:............................
