Charity place application

Before completing this application form, please note that we are not able to grant charity places in full or half marathons to individuals who have been diagnosed with cardiomyopathy or who have an ICD or other device fitted. We have made this decision based on comprehensive medical advice from senior cardiologists and it is in place to ensure that no individual with cardiomyopathy puts themselves at risk whilst raising funds for Cardiomyopathy UK. Read more about our policy here.

Due to the inherited nature of many cardiomyopathies, if you have a blood relative with cardiomyopathy, to grant a charity place in a full or half marathon, we require: 

  • you to have had cardiac screening (an ECG and echo) within 1 year of the event with no concerns raised 

  • you to have had a conversation with your cardiologist (or the healthcare professional who conducts your screening and is responsible for your care) about your intentions to take part in the event and sign off from the cardiologist that it is safe for you to take part  

If you have not had screening within this timeframe, we will hold a place in the event for you whilst you seek screening. This may include carrying over your place to the same event the following year. 

We understand that other charities may not have these requirements. Given the nature of our cause it is important that we support our charity runners to minimise any risks to their heart whilst undertaking these events for Cardiomyopathy UK.

Charity Event Place Terms and Conditions:
Participation
  1. I will inform the fundraising team immediately if I have to withdraw from or cancel my place due to injury or other serious circumstances. 
  2. I agree that if I withdraw for any reason prior to the event, any monies raised will be non-refundable and will be treated as a donation to Cardiomyopathy UK. I will make donors aware of what happens to their donations if I fail to complete the event.  
  3. I understand that if I withdraw for any reason prior to the event, Cardiomyopathy UK cannot guarantee to grant me a place in future events. 
  4. Where I am aware of cardiomyopathy or a related condition affecting myself or a family member, or if I have a cardiac device fitted, I will comply with Cardiomyopathy UK’s fundraising safely process and requirements before participating in the event.
  5. I understand that Cardiomyopathy UK may be unable to grant me a place and reserve the right to revoke my place if upon medical guidance, the Charity does not feel it is safe for me to participate.
  6. I understand that Cardiomyopathy UK reserves the right to revoke my event place in exceptional circumstances.
  7. I understand that this event is organised by the event organiser who is solely responsible for the conduct and operation of the event, including all health and safety requirements. I understand that my participation in the event is conditional upon the event organiser’s terms and conditions.
  8. I understand that Cardiomyopathy UK is not responsible for the event and accepts no liability whatsoever in relation to my participation in it.
  9. I understand that Cardiomyopathy UK will contact me by phone, email and post to provide everything I need for my chosen event, including fundraising tips and advice, useful updates about the event and training support.
  10. I confirm that I will be at least the minimum age required by the event organiser at the date of the event.

    Fundraising activity
  11. I agree to raise the minimum sponsorship amount for Cardiomyopathy UK (*excludes any gift aid) by the fundraising deadline, which is 6 weeks after the event date. 
  12. I will only use lawful means to fundraise and will not do anything that is likely to bring Cardiomyopathy UK into disrepute.
  13. If any funds intended for the charity are raised and paid to me, I will pay it directly to Cardiomyopathy UK through the payment methods they suggest. 
  14. I will abide by the Fundraising Regulator’s Code of Fundraising Practice
  15. I will show the charity any material bearing the Cardiomyopathy UK name or logo, or that discusses the work of the organisation, prior to printing and distribution. 
     
Personal data and images
  1. I give permission for the free use of my first name, picture, image, feedback and comments in any future fundraising materials and/or promotion conducted by Cardiomyopathy UK.
  2. I give permission for my personal information to be stored and used by Cardiomyopathy UK in connection with the organisation and administration of the Event and passed to the Event organisers as required. I understand that Cardiomyopathy UK will process my data in accordance with their Privacy Policy.
